Having worked in various restaurants for about a decade putting myself through school and also funding my teenage misadventures, I can tell you that I have never seen the first but I have seen a lot of the second (not washing hands in the toilet or anywhere for that matter; gloves were also unheard of).
Just reread. I am glad nobody washed their hands in the toliet but they also did not appear to be washing them in the restroom either. (Cannot absolutely confirm but I usually did not hear running water in there)
This is why I don't go to Wendys anymore. Its been a few months but when we got home hubbys sandwich was thrown in the bag in pieces, the wrapper laying on top of it and the fries were dumped on top of the mess. I am thinking maybe Helen Keller or po'd employee either way- won't go back to one.

No matter what it looked like, just hope they didn't spit in it. And that they washed their hands after going to the toilet.
